Immunisation History Statements

Immunisation History Statements are available from both the Australian Immunisation Register (AIR) and the Human Papillomavirus Register (HPV Register). Immunisation History Statements may be necessary for child care and school enrolment, employment at certain workplaces and are currently used by Medicare and Centrelink to determine eligibility for some family assistance payments.

Australian Immunisation Register

AIR Statements contain details of vaccines included in the National Immunisation Program (NIP) and most privately purchased vaccines, given to people of all ages. The list of vaccines AIR can record can be viewed on the Department of Human Services’ (DHS) website at Vaccinations accepted in the AIR.

Statements are automatically generated upon completion of the childhood vaccination schedule (usually around four years of age) and are mailed to the address most recently recorded by Medicare for that individual. Parents of children under 14 years old and young individuals between 14 and 20 years can obtain an AIR Statement in the following ways:

Additionally parents, guardians or a legal representative of an individual can also request an Immunisation History Statement for individuals incapable of managing their own health affairs.

Vaccinations given through school program may not appear in this statement.

HPV Register

The HPV Register contains details of HPV vaccinations administered through schools since 2007. The HPV Register automatically sends a Vaccination History Statement to the vaccine recipient once they have received the full course of the HPV vaccine. Statements can be obtained by:
